That was certainly creative. Alfred being a good prince and all, until Matthew stole his lover away and then died...or was killed with said lover for cheating, as would probably be the law in some places. (The whole "It would be longer before Alfred would be able to forgive himself", part was what made me think Matthew and the stolen lover might have been executed.) I think he just died shortly after in some sort of accident or of a disease or something, and that was part of the hitch. Anyway, I totally didn't see the ghost twist coming.
I love how Arthur was so clever at how he went about it. Living I mean. I think after the first couple of months Alfred probably started to realize what was going on, but let it continue regardless. Actually, I think he caught Alfred's attention that first night even, with his abruptness and want of a conversation. I was kind of hoping that the whole "killed the other people called to his bed at night" would be avoided in this tale, but I can see it had a bit of a darker twist anyway. Meaning that Alfred was still a prince and somewhat cold and hard because of it. Also, him becoming cold and hard after being hurt the way he was, sadly, would have started him on the path of imprisoning/not trusting/possibly executing his other one-night-stand type lovers.
The ending was adorable though. The tone changed quickly in between dark inclinations, the sullenness of the atmosphere left by Matthew's past actions, and the actual, almost cheerful and friendly interactions between Alfred and Arthur. The ending was adorable. It was a good, very well thought out story. I applaud you bringing a new theme into the Hetalia universe. Nice going. It worked out really well.
